Seattle Assisted Living Network is a placement service — we help families and healthcare professionals find the right adult family home for patients with high-acuity care needs.
Unlike general placement services, we specialize in complex cases: patients who require skilled nursing care, specialized medical equipment, or 24/7 monitoring. We've built a network of adult family homes operated by Registered Nurses and facilities with Private Duty Nursing contracts specifically to serve this population.
Our service is free for families. Adult family homes pay us a placement fee when we successfully match a patient to their facility.

The Placement Process

Free 30-minute call to understand the patient's medical needs, care requirements, and placement preferences.
2. Matching
We search our network and identify 3-5 adult family homes that can accommodate the specific care needs.
3. Tours
We coordinate facility visits (in-person or virtual) and help evaluate options.
4. Placement
We handle paperwork, coordinate the move, and ensure a smooth transition to care.

Understanding Adult Family Home Care Levels
Washington State licenses adult family homes (AFHs) to provide different levels of care. Understanding these distinctions helps families and healthcare professionals identify appropriate placement options. According to the Washington State Department of Social and Health Services (DSHS), adult family homes must meet specific requirements for each care level they provide.
Basic Care
Most adult family homes provide basic care services including assistance with activities of daily living (ADLs), medication management, meal preparation, housekeeping, and personal care. These homes serve residents who need help with daily tasks but don't require skilled nursing care.
Specialized Dementia Care
Adult family homes providing memory care must meet additional requirements established by DSHS, including secured environments, specialized staff training, and dementia-specific care protocols. These homes serve residents with Alzheimer's disease and other forms of dementia who may wander or need behavioral support.
Skilled Nursing Care
Some adult family homes are operated by Registered Nurses (RNs) or Licensed Practical Nurses (LPNs) who can provide skilled nursing services. These homes can handle complex medical needs including wound care, IV therapy, and medication administration that requires nursing judgment.
Private Duty Nursing (PDN)
The highest level of care in adult family homes involves Private Duty Nursing — one-on-one skilled nursing care funded through Medicaid for eligible patients. Homes with PDN contracts can accept ventilator-dependent patients, those with tracheostomies requiring frequent suctioning, and other critically ill individuals who would otherwise require hospital or nursing facility care. The Washington Health Care Authority administers the PDN program for Medicaid beneficiaries.
