Hours & Availability

We understand that placement needs don't always happen during business hours. According to the American Hospital Association, hospital discharge planning often requires rapid coordination with post-acute care facilities. That's why we prioritize accessibility for both families and healthcare professionals.

When You Can Reach Us

  • Phone (7 days/week): We answer calls and respond to urgent situations every day, including weekends and holidays. Hospital discharges don't wait for Monday morning.
  • Email Response: Monitored during business hours (8 AM - 6 PM Pacific). We respond to all inquiries within one business day, often within hours.
  • Urgent Placements: Call us directly. If your situation is time-sensitive — hospital discharge deadline, family crisis, or immediate care need — we'll prioritize your case.
  • After-Hours Emergencies: Leave a voicemail with "urgent" in your message, and we'll return your call as soon as possible, even outside business hours.

Frequently Asked Questions About Contacting Us

Is there really no cost to families?

Correct — our placement service is completely free for families. Adult family homes pay us a referral fee when they accept a resident we've placed. This means you receive expert guidance at no cost whatsoever. We're motivated to find the right match, not just any match, because our reputation depends on successful long-term placements.

What information should I have ready when I call?

The more details you can provide, the faster we can find appropriate options. Helpful information includes: the patient's primary diagnosis, specific care needs (ventilator, trach, dialysis, etc.), mobility and cognitive status, preferred locations, payment source (Medicaid, private pay, insurance), and placement timeline. Don't worry if you don't have everything — we can work with whatever information you have.

Can I contact you before a hospital discharge?

Absolutely — in fact, we encourage it. Planning ahead gives us more time to find the best options and allows for thoughtful decision-making rather than rushed choices. The Centers for Medicare & Medicaid Services emphasizes that successful care transitions require coordination between hospital teams and post-acute providers.

Do you help with out-of-state transfers?

Yes, we regularly work with families relocating loved ones to the Seattle area from other states. We understand the additional complexity of long-distance moves and can coordinate with out-of-state hospitals, family members, and transportation services.

What if I'm not sure what type of care my loved one needs?

That's exactly what we're here for. Many families contact us feeling overwhelmed and uncertain about care options. We'll help you understand the differences between adult family homes, assisted living, and nursing facilities, and determine what level of care is appropriate based on your loved one's specific situation.

Can discharge planners contact you directly?

Yes — we work extensively with hospital discharge planners, social workers, and case managers. We understand the pressure of bed utilization targets and can move quickly on urgent cases. Call our direct line or use the referral form, and indicate that you're a healthcare professional for priority handling.
