About Seattle Assisted Living Network
We exist to solve one of healthcare's most frustrating problems: finding appropriate care for patients with complex medical needs.
Our Mission
Every day, hospital discharge planners and families face the same challenge: a patient needs skilled care, but facility after facility says "we can't take that patient." Ventilator-dependent? Tracheostomy? Bariatric? The rejections pile up while hospital beds are needed and families grow desperate.
We started Seattle Assisted Living Network to solve this problem. By building a specialized network of adult family homes operated by Registered Nurses and facilities with Private Duty Nursing contracts, we can place patients that other agencies can't handle.
Our mission is simple: We place the patients others can't.
Why We Focus on High-Acuity Care
The adult family home industry is fragmented. Most placement services are generalists — they know a little about a lot of facilities. That works fine for straightforward cases. But when a patient has complex medical needs, generalist services hit a wall.
We chose to specialize because:
The Problem We Solve
- High-acuity patients are the hardest to place — They require specialized equipment, skilled nursing, and facilities willing to accept the responsibility.
- Generic services can't help — Most placement agencies don't have relationships with PDN-capable facilities or RN-operated homes.
- Time is critical — Hospital discharge timelines are tight, and every day without placement costs money and creates stress.
- Families are desperate — After multiple rejections, families don't know where to turn. We become their last resort — and we come through.
Our Approach
Rigorous Vetting
We personally visit every adult family home in our network. We verify licenses, check references, assess capabilities, and conduct ongoing quality monitoring. Not everyone who wants to be in our network gets in.
Relationship-Based
We build personal relationships with discharge planners, case managers, and adult family home operators. When you call us, you're talking to someone who knows your name and understands your needs.
Urgency-Driven
We treat every placement like the urgent situation it is. Hospital beds are needed, families are stressed, and patients need care. We move fast without cutting corners.
Always Accessible
When a discharge planner calls with an urgent case at 4pm on a Friday, someone answers. We provide direct access — no call centers, no phone trees, no voicemail runarounds.
Who We Serve
Hospital Discharge Planners
You have patients who need to be discharged but no one will take them. We become your go-to resource for complex placements.
Hospital Social Workers
You're helping families navigate impossible situations. We take the placement burden off your plate so you can focus on support.
DSHS Case Workers
You're placing Medicaid patients, often with complex needs and limited options. We have PDN-capable homes that accept Medicaid.
Families
You've been told "we can't help" by other agencies. Your loved one has complex needs and you're running out of options. We can help.
Our Values
What We Believe
- Quality over quantity — We'd rather place 20 patients well than 200 patients poorly. Every placement reflects on us.
- Trust is everything — Discharge planners refer to us because they trust us. Families choose our recommendations because they trust us. We never take that for granted.
- Patients first — We match patients to facilities based on care needs, not based on who pays the highest fee. The right placement is the one that works for the patient.
- Accountability — We follow up. If a placement isn't working, we want to know. We advocate for patients and families even after move-in.
- Urgency matters — We understand that every day without placement is a day of stress, cost, and uncertainty. We move fast.
